Lactation consultations are a covered service under most insurance plans, the list of plans above is not a guarantee of benefits or coverage. For telehealth visits, please be aware that many plans require coinsurance for telehealth visits, and some in network plans may not have any telehealth coverage. Please verify your insurance plan coverage details by calling the member services number on your card prior to our visit and provide them with NPI number 1548706302 or NPI 1255898268 to verify in network coverage. If your insurance requires precertification for specialists (some HMO’s require this), you must obtain precertification from your insurance company prior to our visit or our visit may not be covered. Any copays or coinsurance are the clients responsibility and the client will be billed accordingly. For any insurances in which Love and Wellness Lactation, Ltd. is not an in network provider, a deposit payment is due at the time of service, please see deposit fee amount listed under each service. A superbill will be provided for the client to submit to their insurance company to seek reimbursement. Any amounts reimbursed over your self pay amount are to be paid to Love and Wellness Lactation, Ltd.
